New Ultra Protector Series Application 

Effective immediately in AK, AR, AZ, CA, CO, DC, DE, FL, GA, IA, ID, IN, LA, MA, MD, MI, MO, NC, ND, NH, NJ, NM, OH, OK, PA, RI, SC, SD, TN, TX, UT, WI, WV, & WY, please begin using application series 5099(06/11) found in client brochure series 10-170-2 (08/11) - state variations apply, see for more information.


Old applications received after September 15, 2011 will be returned.


10 Things To Get Paid Quicker!

Did you know that most Ultra Protector Series applications are placed within 3 days*?  In combination with daily EFT commission payments, this means you can get paid in record time!


Common mistakes to avoid:

  1. Replacement Information- be sure to include the name of the company with which the insurance is in force.
  2. Beneficiary Information - designation & percentages need to be clearly indicated.
  3. Health Questions - all questions must be completed, including question 6b. Include build and smoking information on page 2. (UP I & II only)
  4. Legibility Issues - clearly write everything on the application; this reduces turnaround time.
  5. Modal Premium- when calculating modal premium, indicate client's birthday versus the effective date. If effective date is past the birthday, the age is not saved and an increase in premium will occur.
  6. Agent's Report Page - replacement questions on Agent's Report page must match answers given to replacement questions on page 2.
  7. Effective Date - provide the month with the effective date.
  8. Accelerated Benefit Payment Rider - do not send this form when applying for UPII or III; otherwise there will be an amendment.
  9. Bank Draft Form - signature of account holder or person listed on voided check is needed on the form.
  10. Page 3 of Application - both the signed city and state must be included on page 3.
